The membership form defines what your members fill in and what they pay. It is set up in three tabs: **General** (name, activation, payment methods), **Form** (the fields) and **Help**. The membership amount is built from one or more **Paying option** fields.

To open the editor, go to **Manage the club** > **Configure sports seasons**, then tap a form's edit icon or **Add a form** on the season card (see [How do I create and manage the club's seasons?](/en/docs/members/seasons/)).

## General tab: name and activation

- **Name**: the form's name, shown to members (for example "Adult membership 2026-2027").
- **Is active ?**: only active forms are offered to members. Deactivate a form to withdraw it without deleting it.

## General tab: payment methods

Under **Payment of membership amount**, switch on one or more methods:

- **Can pay online**: the member pays by card in the app. Then choose the **Billing of costs CaniClub Connect**:
  - **Paid by the member**: the fees are added to the membership amount and the club receives the full amount;
  - **Offered by the club/company**: the fees are included and the club receives the membership amount minus the fees.

  Enter the **IBAN** and **BIC** of the account that will receive the membership fees, and the **Emails** that will receive the invoice when the money is transferred. The amount is paid into that account within 15 days of joining.
- **Can pay with bank Check**: enter the cheque's payee (**Order**) and the postal **Address** to send it to.
- **Can pay with transfer**: enter the **IBAN** and **BIC** of the account to pay into.

> **NOTE**
Online payment fees are €1 for a membership under €15 and 8% above. Cheques and bank transfers are free. See [Pricing](/en/pricing/).

## Form tab: add and edit fields

1. Tap **Add a field** at the bottom of the list.
2. Tap the field to open its editor. It has three tabs:
   - **Field**: the **Type** and the **Label** (plus the **Amount** for a paying option);
   - **Options**: **Is required**, the **Initial value**, the **Choices** of a list and a **Description**;
   - **File**: **Attach a file ?** to attach a document to the field (club rules, for example), with a label.
3. Drag the fields to reorder them.
4. To remove a field, open it and tap **Delete**.
5. Tap **Save** in the action bar.

## Which field types should I use?

| Type | Use |
| --- | --- |
| **Header** | Title grouping the following fields, with an optional description |
| **Checkbox** | Yes or no (accepting the rules, photo consent…) |
| **Text** | Short text, such as a name |
| **Multilines text** | Long text |
| **Integer** | Whole number |
| **Float** | Number with or without decimals |
| **Radio box** | One value from a list; type the **Choices** separated by ";" |
| **Amount (Allow user to make a donation)** | The member enters an amount |
| **Paying option (Invoiced like membership)** | Option ticked or not, with its **Amount** |
| **Image** | Shows an image in the form |
| **File** | The member uploads a file (medical certificate, photo…) |

## How do I set the membership amount?

The amount to pay is the sum of the **Paying option** fields ticked by the member, the paying options marked **Is required** and the free amounts the member types in **Amount** fields. For example:

- "Club membership": required paying option, €40;
- "Federation licence": optional paying option, €30;
- "Second dog": optional paying option, €10.

If no paying option is ticked, the membership is free: the request goes straight to the club with no payment step.

> **CAUTION**
Only **Paying option** fields count towards the amount to pay. For a fixed fee, use a paying option marked **Is required**.

## Check the form

From the season card, **Open** shows the form as your members will see it. The editor's **Help** tab sums up how it works and shows a sample form with a required paying option and optional ones.
